Documented
Nurse Jonathan Roberts records an entry in my medical notes that is factually incorrect and appears to contain fabricated information presented as fact.
On 1 September 2021, following my first consultation, Nurse Jonathan Roberts recorded the following statement in my medical notes:
“He was ‘found’ to be taking his mother’s Oramorph painkiller medication, and he knows it was wrong.”
This statement is inaccurate and misleading. I was not “found” to be taking my mother’s Oramorph, nor had I admitted to doing so.
The same letter records that my mother was receiving hospice care for cancer. Importantly, my mother was allergic to Oramorph, and this was recorded on her medical allergy list.
The wording used by Jonathan Roberts therefore presents a serious allegation as an established fact when it was not. Anyone who reads that statement would assume that I was taking my mum's pain medication while she was dying.
I dispute both the suggestion that I was found taking my mother’s medication or the implication that I had acknowledged wrongdoing.
I've been going to this clinic since 2022 and never felt comfortable or welcome. I only found out in March 2026 that this was in my notes.
